• The capitalized first letter of your first name
• The first letter of your last name and
• Your six-digit birthdate.

For example, if your name is John Smith and you were born on January 2, 1981,
your login would be Js010281

Your password will be: Bugeye1

This portal can be used to review and verify:

• your contact information
• medical history
• medications
• allergies
